How to Become a MediLeaf Member

To be eligible to become a Member, you must have a Doctor's Recommendation and/or permit and one of the following approved California Medical Cannabis Recommendations:

• Patient Recommendation Letter
• Care Giver Recommendation Letter *

You may visit any one of our three locations in Gilroy, San Jose or Willow Glen and present your state approved I.D. and doctor's recommendation.

Your Doctor's Recommendation will be verified through a two-step process before granting membership to the MediLeaf Medical Cannabis Collective Dispensary. The managers of the dispensary are committed to observing and meeting all legal requirements as set forth by the Attorney General of the State of California.

Notice: Each member applicant is required too complete the membership process before he or she may obtain medication from the MediLeaf Medical Cannabis Collective Dispensary,  all members are required to:

• Present original documentation for verification and approval
• Sign the Patient membership form
• Agree to abide by the  Rules and Regulations  

* (Along with the Patient Recommend Letter of the Patient for which you are providing care.)
